Please completely fill out the paperwork and send back to our office.
When our office receives your paperwork, we'll call you to schedule
your appointment or we'll send you a postcard containing all your
new appointment information.

The day of your appointment, please bring your insurance card(s)
with you. If your insurance does not cover the visit or our office
does not participate with your insurance, you will be responsible
at the time of service for your visit.

Also, if possible, please call your previous doctor(s), if any, and
request records to be sent to our office prior to your appointment.
Please ask for, at least, your last 2 years of pap smears, mammos,
office visits and any sonograms surgeries or gynecological
problems you may have had. This information will be very helpful
at the time of your appointment.

If you have Medicare for Insurance our office will bill Medicare
on your behalf.
