Please fill out the enclosed paperwork and send completed back to
our office. When we receive your paperwork our office will call you
to schedule your appointment. The day of your appointment please
bring your insurance card or cards with you. If your insurance does
not cover the visit or our office does not participate with your
insurance, you will be responsible at the time of service for your visit.
Also, if you could please call your previous doctor(s), if any,
and request records to be sent to our office prior to your
appointment. Please ask for at least your last 2 years of pap smears,
mammos, office visits, any sonograms surgeries or gynecological
problems you may have had. This information will be very helpful
at the time of your appointment.
If you have Medicare for Insurance our office will bill Medicare
on your behalf.
